Chief Complaint
Patient scheduled for removal of impacted #48
Clinical Findings
Impacted #48 in close proximity to the inferior dental nerve (IDN).
CBCT shows compression of the IDN against the lingual cortical plate by the roots of #48.
No signs of infection or pathology associated with the impacted tooth.
Full dentition noted.
Separators placed interproximally at all first molars in preparation for orthodontic treatment.
Occlusion digitally recorded.
Upper and lower arches scanned.
Intra-oral and profile photographs taken
Treatment Plan
Surgical removal of impacted #48 under local anaesthesia.
Continue with orthodontic documentation.
Informed patient of surgical risks, especially potential paraesthesia due to proximity to IDN.
Post-operative CBCT to confirm complete removal and assess anatomical integrity.
Pre-operative Assessment
CBCT reviewed.
Thorough explanation of risks, especially possible paraesthesia of lower lip and tongue.
Patient demonstrated understanding and consented to the procedure.
No systemic contraindications to surgery.
Anesthesia
Local anaesthetic block administered: Right Inferior Dental Nerve (IDN), Lingual Nerve (LN), and Long Buccal Nerve (LBN).
Surgical Procedure
Patient cleaned and draped under sterile conditions.
Buccal incision made, and flap raised.
Bone guttering performed to expose the tooth.
Tooth sectioned and elevated in 10 pieces.
Site curetted and irrigated thoroughly with saline.
5 units of BSS (Biodegradable Surgical Sponge) placed for primary closure.
Hemostasis achieved.
Post-operative instructions given.
